Case Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Deliver comprehensive case management services to frail elderly individuals and adults with intellectual, developmental, or physical disabilities within the Wisconsin Family Care program.

• Evaluate members' health and safety requirements, strengths, interests, and preferences.

• Create detailed Member Care Plans (MCPs).

• Collaborate with a Field Care Nurse to meet members' health and safety needs.

• Travel to members' locations for in-person social assessments upon enrollment and at least every six months.

• Conduct quarterly in-home visits to members and maintain monthly phone communication.

• Organize support services related to social integration, community resources, employment, housing, and other non-medical needs.

• Ensure the delivery of cost-effective services.

• Assess risk factors and provide education to members.

• Keep precise case notes, service authorizations, and updates to MCPs.

• Report to the Manager of Care Coaching.

• Assist members in Taylor/Price County and adjacent areas.

⛳️ Requirements

• Bachelor's degree in human services or a related discipline, with a minimum of 1 year of experience working with frail elderly individuals or adults with intellectual, developmental, or physical disabilities; alternatively, a Bachelor's degree in a different field with at least 3 years of such experience.

• Willingness to travel up to 40% of the time to meet members.

• Valid state driver's license required.

• Proof of personal vehicle liability insurance with minimum limits of 100/300/100.

• Preferred residence within 45 minutes or less of the designated coverage area.

• Experience in Case Management is preferred.

• Familiarity with electronic case note documentation is preferred.

• Knowledge of community health and social service agencies, along with additional community resources, is preferred.

• The role is patient-facing, necessitating TB screening.

• Home internet with a minimum download speed of 25 Mbps and upload speed of 10 Mbps is required for Home or Hybrid Home/Office employees.

• A dedicated workspace free from ongoing interruptions to safeguard member PHI/HIPAA information is essential.
