Case Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Evaluate the health and safety needs of members while identifying their strengths, interests, and preferences.

• Create detailed Member Care Plans (MCPs).

• Collaborate with a Field Care Nurse to meet the health and safety requirements of members.

• Deliver support in the least restrictive environment in accordance with the MCP.

• Perform face-to-face social assessments during enrollment and at least every six months, usually at the members' homes.

• Conduct quarterly in-person visits and maintain monthly phone communication.

• Organize support services related to social integration, community resources, employment, housing, and other non-medical necessities.

• Ensure the delivery of cost-effective services.

• Assess risk factors and provide education to members.

• Keep precise case notes, service authorizations, and updates to the MCP.

• Support frail elders and adults with intellectual, developmental, or physical disabilities in the Wisconsin Family Care program.

• Be prepared to travel up to 40% within the designated coverage area.

⛳️ Requirements

• A Bachelor's degree in human services or a related field, along with at least 1 year of experience working with frail elders or adults with intellectual, developmental, or physical disabilities.

• Alternatively, a Bachelor's degree in another discipline and a minimum of 3 years of relevant experience.

• A valid state driver's license.

• Evidence of personal vehicle liability insurance with limits of at least 100/300/100.

• Preferred residency within 45 minutes or less of the assigned coverage area.

• Preferred case management experience.

• Familiarity with electronic case note documentation is preferred.

• Knowledge of community health and social service agencies, as well as additional community resources, is preferred.

• Capability to meet home internet requirements of at least 25 Mbps download and 10 Mbps upload.

• Ability to complete TB screening for this patient-facing position.
