Care Navigator

This is a fully remote position, open to applicants in United States.

📋 Description

• Address inquiries from patients and those related to durable power of attorney (DPOA) via telephone.

• Respond to messages from patients or caregivers received through the Curana Patient Portal.

• Assist patients in scheduling follow-up appointments with Curana Providers or specialists.

• Manage electronic health records (EHR) and ensure that patient records are accurate and up-to-date.

• Coordinate necessary documents for provider review or signature.

• Facilitate provider orders and escalate any findings as needed.

• Maintain patient rosters for individuals enrolled in APCM and GUIDE.

• Support the scheduling of providers.

• Assist with prior authorizations.

• Obtain patient records and diagnostic test results.

• Professionally and courteously manage incoming calls.

• Collaborate with the Curana Interdisciplinary Care Team to ensure effective communication within the health network.

• Perform additional duties as assigned.

⛳️ Requirements

• High school diploma or equivalent qualification.

• At least 1 year of experience in a medical office, Senior Living Community engagement, or other relevant fields.

• Minimum of 1 year of experience in Electronic Health Record (EHR) documentation or other practice management tools.

• Comprehensive understanding of medical terminology.

• Ability to interpret medical records, lab results, and appointment notes.

• Basic knowledge of reviewing patient screening tools and identifying changes over time.

• Capacity to work in a distraction-free environment.

• Excellent organizational and time management skills with the ability to prioritize tasks effectively.

• Proficient in handling multiple tasks simultaneously.

• Strong computer skills and adaptability to various technology platforms.

• Prior experience with virtual triage is preferred.

• Bilingual or multilingual communication skills are preferred.

• A reliable high-speed internet connection is required.
