This is a fully remote position, open to applicants in California.

📋 Description

• Execute care management responsibilities to evaluate, plan, and coordinate medical and support services for members post-discharge.

• Conduct medication reviews and reconcile medications from pre-admission and post-discharge.

• Identify transition support services in collaboration with care management and coordination teams.

• Perform post-discharge assessments for members moving from healthcare facilities.

• Create care/service plans and work alongside discharge planners, providers, specialists, and interdisciplinary teams.

• Evaluate member health status, resource needs, services, and treatment plans, and implement necessary interventions.

• Facilitate the transition into active care management based on individual member requirements.

• Provide educational materials and resources to members, caregivers, and healthcare providers.

• Coordinate services among primary care providers, specialists, medical personnel, and non-medical resources.

• Conduct outreach through telephonic, digital, home, and/or site visits.

• Document member details and care management activities to ensure compliance with regulatory standards.

• Offer leadership insights on potential improvements for cost-effective care and quality service delivery.

• Carry out other assigned tasks and adhere to policies and standards.


⛳️ Requirements

• Valid RN license in the state of California.

• Degree from an accredited nursing program or a Master's degree in Behavioral Health or Social Work.

• 2–4 years of relevant experience.

• Licensure as RN, LISW, LCSW, LMSW, LMFT, LMHC, or LPC is required.

• Preference for experience in case management, transitional care, care coordination, and discharge planning.

• Preference for capability to work Pacific Time (PST) hours.

• Must comply with current state, federal, and third-party payer regulations.
