This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Evaluate, strategize, and organize medical and ancillary services for members after discharge.

• Conduct medication assessments and reconcile medications before and after discharge.

• Collaborate with care management and coordination teams to pinpoint transition support services.

• Perform post-discharge evaluations for members moving from healthcare facilities.

• Create care/service plans and work in conjunction with discharge planners, providers, specialists, and interdisciplinary teams.

• Analyze member health conditions, resource requirements, services, and treatment strategies while providing necessary interventions.

• Smooth transitions into active care management.

• Offer or facilitate educational resources and materials for members, caregivers, and healthcare providers.

• Coordinate services among primary care providers, specialists, medical professionals, and non-medical resources.

• Execute outreach via telephone, digital platforms, home visits, or other locations.

• Gather, document, and preserve member data and care management activities.

• Provide leadership insights on care and opportunities for quality improvement.

• Undertake additional assigned responsibilities and adhere to established policies and standards.


⛳️ Requirements

• A Master's degree in Behavioral Health or Social Work, or a degree from an accredited nursing program.

• 2 to 4 years of relevant experience.

• Active clinical licensure in Wisconsin.

• Required licenses include LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN.

• Experience in case management is essential.

• Extensive knowledge of community resources.

• A background in behavioral health.

• Adherence to current state, federal, and third-party payer regulations.

• Must fulfill applicable work authorization and legal prerequisites.
