This is a fully remote position, open to applicants in Alabama, +19 more states.
β’ Accountable for the comprehensive management of care and ensuring high-quality services for participants.
β’ Evaluate assessments and documentation specific to each discipline for quality, addressing any deficiencies with field staff.
β’ Engage in and advocate for the social work discipline during care planning meetings.
β’ Oversee documentation and interventions carried out by field staff.
β’ Facilitate coaching, competency development, and training sessions for field personnel as necessary.
β’ Organize a continuum of care that aligns with the healthcare needs and goals of members.
β’ Arrange, coordinate, and authorize member-specific services such as assistance with ADLs/IADLs, housing, meals, transportation, and community-based services.
β’ Execute care management activities and interventions to achieve participant objectives.
β’ Collaborate with team members, facilities, discharge planners, and home care coordinators during transitions in care settings.
β’ Document services in accordance with CLHC standards and federal/state regulations.
β’ Coordinate long-term care services in nursing homes, rehabilitation facilities, and other environments.
β’ Work with PCPs, specialty physicians, and the interdisciplinary team regarding changes in participant conditions and resource coordination.
β’ Provide or arrange for skilled services, manage service authorizations, conduct periodic reassessments, and evaluations.
β’ Monitor care activities, services provided, member responses, plan effectiveness, and service utilization; adjust plans as necessary.
β’ Keep current with health-related issues, procedures, evidence-based guidelines, medications, and practice standards.
β’ Suggest and contribute to enhancements in services, programs, policies, and procedures.
β’ Adhere to disciplinary policies and collaborate with Human Resources regarding disciplinary actions and terminations.
β’ Operate within the scope of authority to practice and fulfill organizational competencies prior to working independently.
β’ Execute additional duties as assigned.
β’ Maintain a schedule of 8:00 AMβ5:00 PM, totaling 40 hours per week.
β’ Graduated from a Master of Social Work program recognized by the New York State Education Department (NYSED).
β’ At least two (2) years of administrative experience in a management role within a certified home health agency (CHHA), Managed Care, long-term home health care (LHCSA), acute care, medical-surgical, critical care, nursing home experience, or diagnostic & treatment clinic preferred.
β’ Customer service experience is mandatory.
β’ Experience managing long-term care insurance is advantageous.
β’ A minimum of one (1) year of experience working with a frail or elderly population; if the individual possesses less than one (1) year of experience but meets all other criteria, they must receive proper training from the PACE organization on working with a frail or elderly population upon hiring.
β’ Supervisory experience is preferred.
β’ Must be legally authorized (e.g., currently licensed, registered, or certified if applicable) to practice in the state where the healthcare professional will perform their duties.
β’ Must be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
β’ A current active and unrestricted license and registration in New York State is required.
β’ Bilingual skills are preferred.
β’ Ability to meet the specified physical, visual, auditory, motor, and cognitive requirements.
