Care Manager – Social Work

This is a fully remote position, open to applicants in Alabama, +19 more states.

📋 Description

• Accountable for the comprehensive management of care and ensuring high-quality services for participants.

• Evaluate assessments and documentation specific to each discipline for quality, addressing any deficiencies with field staff.

• Engage in and advocate for the social work discipline during care planning meetings.

• Oversee documentation and interventions carried out by field staff.

• Facilitate coaching, competency development, and training sessions for field personnel as necessary.

• Organize a continuum of care that aligns with the healthcare needs and goals of members.

• Arrange, coordinate, and authorize member-specific services such as assistance with ADLs/IADLs, housing, meals, transportation, and community-based services.

• Execute care management activities and interventions to achieve participant objectives.

• Collaborate with team members, facilities, discharge planners, and home care coordinators during transitions in care settings.

• Document services in accordance with CLHC standards and federal/state regulations.

• Coordinate long-term care services in nursing homes, rehabilitation facilities, and other environments.

• Work with PCPs, specialty physicians, and the interdisciplinary team regarding changes in participant conditions and resource coordination.

• Provide or arrange for skilled services, manage service authorizations, conduct periodic reassessments, and evaluations.

• Monitor care activities, services provided, member responses, plan effectiveness, and service utilization; adjust plans as necessary.

• Keep current with health-related issues, procedures, evidence-based guidelines, medications, and practice standards.

• Suggest and contribute to enhancements in services, programs, policies, and procedures.

• Adhere to disciplinary policies and collaborate with Human Resources regarding disciplinary actions and terminations.

• Operate within the scope of authority to practice and fulfill organizational competencies prior to working independently.

• Execute additional duties as assigned.

• Maintain a schedule of 8:00 AM–5:00 PM, totaling 40 hours per week.

⛳️ Requirements

• Graduated from a Master of Social Work program recognized by the New York State Education Department (NYSED).

• At least two (2) years of administrative experience in a management role within a certified home health agency (CHHA), Managed Care, long-term home health care (LHCSA), acute care, medical-surgical, critical care, nursing home experience, or diagnostic & treatment clinic preferred.

• Customer service experience is mandatory.

• Experience managing long-term care insurance is advantageous.

• A minimum of one (1) year of experience working with a frail or elderly population; if the individual possesses less than one (1) year of experience but meets all other criteria, they must receive proper training from the PACE organization on working with a frail or elderly population upon hiring.

• Supervisory experience is preferred.

• Must be legally authorized (e.g., currently licensed, registered, or certified if applicable) to practice in the state where the healthcare professional will perform their duties.

• Must be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.

• A current active and unrestricted license and registration in New York State is required.

• Bilingual skills are preferred.

• Ability to meet the specified physical, visual, auditory, motor, and cognitive requirements.

• Capacity to travel and walk between various sites/locations and work areas throughout the day.
