Care Manager, RN – OB

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Develop, evaluate, and implement intricate care management activities for members primarily with physical needs.

• Assess member needs, obstacles to accessing care, and the social determinants affecting health.

• Create ongoing care plans and collaborate with healthcare providers, specialists, and community resources.

• Identify issues and barriers to care, providing appropriate care management interventions.

• Facilitate coordination among members, families or caregivers, and care provider teams.

• Monitor and follow up on member status, changes in condition, and progress toward care plan objectives.

• Offer resource support for services such as employment, housing, participant direction, independent living, justice, and foster care.

• Ensure timely access to care by facilitating care management and collaborating with providers.

• Conduct outreach via telephonic, digital, home, and/or other locations.

• Gather, document, and maintain member information and care management activities to ensure regulatory compliance.

• Educate members and their families or caregivers about disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.

• Provide leadership insights on opportunities to enhance care quality and cost-effectiveness.

• Adhere to all policies and standards while performing additional assigned tasks.

⛳️ Requirements

• A degree from an Accredited School of Nursing or a Bachelor's degree in Nursing.

• 2–4 years of relevant experience.

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is required.

• Capability to work remotely from any location within the state of Missouri.

• Availability for work Monday–Friday, from 8am to 5pm.

• Must provide education and support to pregnant members.
