This is a fully remote position, open to applicants in Washington.
• Design, assess, and implement intricate care management activities for members with mainly physical health requirements.
• Create individualized care plans and educational resources for members and their families.
• Analyze member needs, access challenges, and social determinants impacting health.
• Formulate care/service plans and collaborate with healthcare providers, specialists, and community organizations.
• Recognize care obstacles and deliver care management solutions.
• Coordinate person-centered care among members, their caregivers, and healthcare teams.
• Track member status, changes in condition, and progress towards care plan objectives.
• Offer resource assistance for employment, housing, independent living, justice, foster care, and additional services.
• Ensure timely access to necessary care and services.
• Conduct outreach via telephone, digital platforms, home visits, and other locations as required.
• Gather, document, and preserve member data and care management activities to meet regulatory standards.
• Educate members and their families/caregivers on disease processes, care gaps, provider directives, care alternatives, referrals, and healthcare benefits.
• Provide constructive feedback on potential improvements to care quality and cost-effectiveness.
• Execute other assigned responsibilities and adhere to established policies and standards.
• Degree from an accredited nursing program or a Bachelor’s degree in Nursing.
• 2–4 years of relevant experience.
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
• WA State RN Licensure and/or Compact State Licensure required.
• Experience with maternal and infant populations is highly preferred.
• Experience assisting members in behavioral health crises is strongly preferred.
• Availability to work Monday–Friday, from 8:00 am to 5:00 pm Pacific Standard Time (PST).
