Care Manager, RN

This is a fully remote position, open to applicants in United States.

📋 Description

• Develop, evaluate, and facilitate intricate care management activities for members primarily with physical needs.

• Assess member requirements, access challenges, and social determinants affecting health.

• Create continuous care plans and collaborate with healthcare providers, specialists, and community resources.

• Identify care obstacles and implement care management interventions.

• Coordinate efforts among members, families/caregivers, and care provider teams to ensure a person-centered approach.

• Monitor member status, changes in condition, and progress towards care plan objectives.

• Offer resource support related to employment, housing, independent living, justice, foster care, and additional services.

• Facilitate prompt access to necessary care and services.

• Conduct outreach through telephonic, digital, home, and other site visits.

• Collect, document, and maintain member information and care management activities to ensure regulatory compliance.

• Educate members and families/caregivers on disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.

• Provide leadership insights on opportunities to enhance care quality and cost-effectiveness.

• Perform other assigned tasks while adhering to policies and standards.

⛳️ Requirements

• Must reside in Georgia.

• Experience in obstetrics (OB) is required.

• Degree from an accredited nursing school or a Bachelor's degree in Nursing.

• 2–4 years of relevant experience is necessary.

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is required.

• Adherence to all policies and standards is mandatory.

• Qualified candidates with arrest or conviction records will be considered in line with the LA County Ordinance and the California Fair Chance Act.
