Care Manager, RN – Case Management

This is a fully remote position, open to applicants in California.

📋 Description

• Oversee the care of specific patients during their hospital stay.

• Ensure the availability of adequate clinical and medical center resources.

• Maintain daily communication with medical staff, medical center personnel, and external resources.

• Guarantee the proper use of resources in line with clinical and financial guidelines.

• Review medical records for discrepancies from clinical and financial standards.

• Intervene to secure optimal outcomes for both patients and the medical center.

• Utilize knowledge of risk management and infection control.

• Support the healthcare team in achieving targeted outcomes.

⛳️ Requirements

• Environmental Orientation must be completed within 30 days of hire.

• Case Management Competency is required within 90 days of hire.

• General Nurse Competency should be completed within 30 days of hire.

• California Registered Nurse License is required upon hire.

• A minimum of 2 years of acute nursing experience, with a preference for progressive clinical experience or case management experience.

• Bachelor’s Degree in Nursing is required upon hire, with a BSN to be obtained within 3 years of the hire date (preferred qualification).

• CCM certification is preferred upon hire.
