Case Manager, RN

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Perform telephonic evaluations to assess members' medical, behavioral, and social requirements.

• Create and manage customized care strategies.

• Collaborate with healthcare professionals and multidisciplinary teams to eliminate obstacles to care.

• Utilize clinical expertise to manage intricate health issues and enhance outcomes.

• Assist members during care transitions and link them to suitable resources.

• Ensure adherence to regulatory and organizational case management standards.

• Work Monday to Friday from 9:00 AM to 5:30 PM EST.

• May occasionally work during evenings, weekends, and holidays.

• Be prepared for limited travel of up to 10%.

⛳️ Requirements

• Active and unrestricted RN license in North Carolina.

• Must be a resident of North Carolina.

• At least 2 years of clinical experience as an RN.

• Associate Degree in Nursing.

• Experience in any clinical environment is accepted, including ICU, NICU, Emergency Department, Med/Surg, Telemetry, OR, Home Health, Hospice, Long-Term Care, Infusion, and Orthopedics.

• Excellent communication and member engagement abilities.

• Direct/hardwired internet connection to a modem/router located within 7 feet of the computer.

• Minimum internet speeds of 25 Mbps for download and 3 Mbps for upload.

• WiFi and satellite internet connections are not allowed.

• A quiet, secure, private home workspace that complies with CVS Health and HIPAA regulations.

• Must work within the state and city of current residence.

• Capability to use multiple monitors and navigate various applications concurrently.

• Proficient in communicating via email, calendar invites, Teams messaging, and virtual meetings.

• Familiarity with Microsoft Office 365 or comparable Google Workspace applications.

• Ability to log into secure systems such as VPN and EHR portals.

• Competence in managing strong passwords and identifying suspicious emails or links.

• Ability to troubleshoot common technical issues independently.

• Preferred: Certified Case Manager (CCM).

• Preferred: Bachelor of Science in Nursing (BSN).

• Preferred: Experience in Case Management or Managed Care.

• Preferred: Experience in Discharge Planning.

• Preferred: Proficiency with Microsoft Office Suite.
