Care Manager, RN

This is a fully remote position, open to applicants in United States.

📋 Description

• Design, evaluate, and implement intricate care management activities primarily for members with physical needs.

• Assess member requirements, barriers to access, and social determinants affecting health.

• Create ongoing care plans and collaborate with healthcare providers, specialists, and community resources.

• Identify issues in care and obstacles, providing appropriate care management interventions.

• Coordinate efforts among members, their families or caregivers, and care provider teams.

• Monitor member conditions, changes in health status, and progress towards the goals set in care plans.

• Offer resource support related to employment, housing, participant direction, independent living, justice, and foster care services.

• Ensure timely access to necessary care and services.

• Conduct outreach through telephonic, digital, home visits, and/or on-site interactions.

• Gather, document, and maintain member and care management information to meet regulatory compliance.

• Educate members and their families or caregivers about disease processes, care gaps, provider instructions, available care options, referrals, and healthcare benefits.

• Provide constructive feedback on opportunities to enhance care quality and cost effectiveness.

• Carry out other assigned responsibilities while adhering to policies and standards.

⛳️ Requirements

• Must be a resident of the state of Georgia.

• Hold a degree from an Accredited School of Nursing or possess a Bachelor's degree in Nursing.

• Have 2–4 years of relevant experience.

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is mandatory.

• Experience in Med/Surg is highly preferred.
