This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Design, evaluate, and implement complex care management activities for members primarily with physical health needs.

• Assess member requirements, obstacles to care, and social determinants impacting health.

• Create ongoing individual care plans and service strategies.

• Collaborate with healthcare providers, specialists, caregivers, and community resources.

• Identify challenges and barriers to care, providing appropriate interventions.

• Coordinate person-centered services and care among members, families, caregivers, and provider teams.

• Monitor and follow up on member status, changes in condition, and progress towards care-plan objectives.

• Offer resource support for employment, housing, participant direction, independent living, justice, and foster care services.

• Ensure timely access to necessary care and services.

• Conduct outreach through telephonic, digital, home visits, and other locations.

• Gather, document, and maintain member information and care management activities to ensure regulatory compliance.

• Educate members and their families/caregivers about diseases, care gaps, provider guidance, care alternatives, referrals, and healthcare benefits.

• Provide constructive feedback on opportunities to enhance care quality and cost-effectiveness.

• Perform additional duties as assigned, adhering to policies and standards.


⛳️ Requirements

• Degree from an accredited nursing program or a Bachelor's degree in Nursing.

• 2–4 years of relevant experience.

• Valid New Jersey Registered Nurse (RN) license is mandatory.

• State Licensure and/or Compact State Licensure is required.

• Preference for candidates who are Spanish-speaking.

• Availability to work Monday through Friday, generally from 8:00 AM to 5:00 PM ET, with flexibility based on member and organizational needs.

• Capability to conduct up to 50% local travel for in-home member visits within the assigned service area.
