This is a fully remote position, open to applicants in New Jersey.
• Develop, evaluate, and implement intricate care management activities tailored for members primarily with physical needs.
• Assess member requirements, obstacles to care, and social determinants impacting health.
• Formulate ongoing care plans and service strategies.
• Collaborate with healthcare providers, specialists, caregivers, and community resources.
• Recognize barriers to care and deliver suitable interventions.
• Coordinate person-centered care and services among members, families, caregivers, and provider teams.
• Monitor member status, changes in condition, and progress towards care plan objectives.
• Provide resource support related to employment, housing, participant direction, independent living, justice, and foster care services.
• Ensure timely access to necessary care and services.
• Conduct outreach through telephonic, digital, home, and various other sites.
• Collect, document, and maintain accurate member information and care management activities.
• Educate members and their families/caregivers on disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.
• Offer leadership insights on opportunities to enhance care quality and cost-effectiveness.
• Execute other assigned responsibilities while adhering to all policies and standards.
• Degree from an accredited nursing school or a Bachelor's degree in Nursing.
• 2 to 4 years of relevant experience.
• Current New Jersey Registered Nurse (RN) license is required.
• RN - Registered Nurse - State Licensure and/or Compact State Licensure is mandatory.
• Capability to serve members throughout Monmouth and Ocean Counties, New Jersey.
• Availability to work Monday through Friday from 8:00 AM to 5:00 PM EST, with flexibility according to member and business needs.
• Willingness to travel locally up to 50% for in-home member visits.
• Ability to independently manage a caseload, coordinate member care, and complete necessary documentation.
