This is a fully remote position, open to applicants in New Jersey.
β’ Design, evaluate, and implement intricate care management initiatives primarily focused on members with physical needs.
β’ Create tailored care plans and provide educational resources for members and their families.
β’ Assess member needs, identify barriers to effective care, and consider social determinants of health.
β’ Formulate continuous care plans and collaborate with healthcare providers, specialists, and community resources.
β’ Recognize issues and obstacles to care and deliver suitable interventions.
β’ Coordinate communication among members, families/caregivers, and care provider teams.
β’ Monitor and follow up on member status, changes in condition, and progress toward the goals outlined in care plans.
β’ Offer resource support for local services such as employment, housing, participant direction, independent living, justice, and foster care.
β’ Facilitate care management and work with providers or specialists to ensure timely access to necessary care.
β’ Conduct outreach through telephonic, digital, home visits, and other locations.
β’ Gather, document, and maintain member information and care management activities to ensure regulatory compliance.
β’ Provide or coordinate education regarding disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.
β’ Offer feedback to leadership on potential improvements in care and quality delivery.
β’ Perform additional duties as assigned and adhere to all policies and standards.
β’ A degree from an accredited nursing school or a Bachelor of Science in Nursing (BSN) is required.
β’ A minimum of two to four years of relevant experience is necessary.
β’ An active RN license in New Jersey is mandatory.
β’ RN - Registered Nurse - State Licensure and/or Compact State Licensure is required.
β’ Must be available to work Monday through Friday, from 8:00 AM to 5:00 PM EST, with flexibility based on member and business requirements.
β’ Willingness to travel locally up to 50% for in-home member visits is essential.
