Care Manager, RN

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Design, evaluate, and implement intricate care management initiatives for members with mainly physical requirements.

• Develop tailored care plans and offer educational resources for members and their families.

• Assess member needs, identify access challenges, and consider social determinants of health.

• Create ongoing care/service strategies and work collaboratively with providers, specialists, and community resources.

• Recognize care challenges and deliver suitable care management solutions.

• Coordinate between members, families/caregivers, and care provider teams to ensure a person-centered approach to care.

• Monitor and follow up on member status, changes in condition, and progress towards care plan objectives.

• Offer local resource assistance for services such as employment, housing, participant direction, independent living, justice, and foster care.

• Ensure timely access to necessary care and services.

• Conduct outreach through telephonic, digital, home, and/or other site visits.

• Gather, document, and uphold member information and care management activities to ensure regulatory compliance.

• Educate members and families/caregivers on disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.

• Provide leadership insights on enhancing care quality and cost-effective service delivery.

• Execute other assigned tasks and adhere to all policies and standards.

⛳️ Requirements

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing.

• 2–4 years of relevant experience.

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is required.

• Availability to work Monday–Friday, 8am–5pm.

• Must be able to work remotely from anywhere within the state of Missouri.
