Care Manager, RN

This is a fully remote position, open to applicants in Mississippi.

📋 Description

• Design, evaluate, and implement intricate care management initiatives for members primarily with physical health needs.

• Assess member requirements, obstacles to access, and social determinants of health, emphasizing priorities identified by members.

• Create continuous care plans and collaborate with providers, specialists, and community resources.

• Recognize challenges and barriers to care, delivering suitable interventions.

• Manage coordination of care among members, families/caregivers, and provider teams.

• Monitor and follow up on member status, changes in condition, and progress toward care plan objectives.

• Offer resource support for services including employment, housing, participant direction, independent living, justice, and foster care.

• Facilitate care management and ensure timely access to necessary care or services.

• Conduct outreach via telephone, digital platforms, home visits, and/or other locations.

• Gather, document, and maintain member details and care management activities to ensure regulatory compliance.

• Educate members and families/caregivers about disease processes, gaps in care, provider instructions, care options, referrals, and healthcare benefits.

• Provide constructive feedback on opportunities to enhance cost-effective care and quality service delivery.

• Execute other assigned responsibilities.

⛳️ Requirements

• A degree from an accredited nursing school or a Bachelor's degree in Nursing.

• 2–4 years of relevant experience.

• RN (Registered Nurse) state licensure and/or compact state licensure is mandatory.

• Must hold a license in Mississippi.

• Residency in Mississippi is required.
