Care Manager – Foster Care

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Design, assess, and implement complex care management initiatives primarily focusing on mental and behavioral health needs for members.

• Assess member requirements through phone calls or in-home visits, recommending or facilitating care/service plans.

• Conduct outreach via telephonic, digital, home, or other site visits.

• Create ongoing care plans for high-acuity members while identifying necessary providers, specialists, and community resources.

• Collaborate with members, families/caregivers, community resources, and care provider teams.

• Monitor care plans, member conditions, treatment side effects, complications, and clinical symptoms.

• Facilitate care coordination and work together with providers and specialists to guarantee timely access to care.

• Gather, document, and maintain member information and care management activities to ensure regulatory compliance.

• Educate members and their families regarding procedures, provider instructions, treatment options, referrals, and healthcare benefits.

• Provide leadership insights on opportunities to enhance care quality and cost-effective service delivery.

• Carry out additional duties as assigned and adhere to policies and standards.

⛳️ Requirements

• Master’s degree in Behavioral Health or Social Work, or a degree from an accredited nursing program.

• 2–4 years of relevant experience.

• Licensed Master’s Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN as per state contract specifications.

• Experience in Behavioral Health is required.

• Capability to work remotely from anywhere within the state of Missouri.

• Availability from Monday to Friday, 8 AM to 5 PM.

• Previous experience in foster care is preferred.
