Care Manager – Dietitian

This is a fully remote position, open to applicants in Alabama, +19 more states.

📋 Description

• Oversee the comprehensive care management and quality of services for participants.

• Evaluate assessments from field staff and discipline-specific documentation for quality assurance and address any deficiencies.

• Engage in care planning meetings to represent the dietitian discipline.

• Supervise field staff documentation and interventions; facilitate coaching, competency evaluations, and training sessions.

• Coordinate a continuous care approach with discipline-specific personnel and the interdisciplinary team (IDT).

• Organize, coordinate, and authorize necessary member-specific services, including assistance with ADLs/IADLs, housing, meals, and transportation.

• Execute care management activities and interventions that align with participant goals.

• Facilitate care coordination across different sites and during transitions between care environments.

• Document services in accordance with CLHC standards and federal/state regulations.

• Oversee long-term care services within nursing homes, rehabilitation facilities, and other settings.

• Collaborate with primary care providers, specialty physicians, specialty services, members, and the IDT.

• Provide or arrange for skilled services, service authorization, periodic reassessment, and evaluations.

• Monitor care management activities, member responses, the effectiveness of the care plan, and service utilization.

• Adjust care plans based on changes in members' health and needs.

• Stay informed about health issues, procedures, evidence-based guidelines, medications, and practice standards.

• Suggest enhancements to services, programs, policies, and procedures.

• Adhere to disciplinary policies and engage Human Resources as necessary.

• Operate within the scope of authority and fulfill PACE competencies before working independently.

• Act as a member of the IDT in an acting capacity.

• Carry out additional responsibilities as assigned.

⛳️ Requirements

• Bachelor's degree in nutrition or a related discipline.

• At least two (2) years of administrative experience in a management role within a certified home health agency (CHHA), long-term home health care (LHCSA), acute care, medical-surgical, and/or critical care; nursing home experience is preferred.

• Required customer service experience.

• Experience in managing long-term care insurance is advantageous.

• Must be legally authorized to practice in the state where the healthcare professional will perform duties.

• Minimum of one (1) year of experience working with a frail or elderly population, or obtain appropriate PACE organization training upon hiring if less than one year.

• Current active and unrestricted license and registration in specialty in New York State is mandatory.

• Must be medically cleared for communicable diseases and have all immunizations current before engaging in direct participant contact.

• Ability to fulfill required physical tasks, including standing, sitting, lifting/pulling/pushing up to 50 pounds, bending/squatting, walking/climbing, fine motor skills, visual accuracy, clear hearing and speaking, and cognitive decision-making.
