Care Manager – Behavioral Health

This is a fully remote position, open to applicants in Oklahoma.

📋 Description

• Evaluate and assess the needs and requirements of members through both telephone and in-person interactions.

• Assist members and their families in navigating suitable care and wellness resources.

• Oversee care, assessments, and evaluations to ensure that members are making progress towards their desired goals.

• Develop comprehensive care plans for members.

• Address behavioral, physical, environmental, and psychosocial health needs utilizing diverse strategies and techniques.

• Identify and tackle obstacles to effective care delivery.

• Work in partnership with providers and community services to enhance quality and cost-effective outcomes.

• Arrange in-person meetings when direct support for enrollees is necessary.

• Collaborate effectively with an interdisciplinary care management team, including community health workers, housing support specialists, SDOH coordinators, and care management support assistants.

• Adhere to established guidelines and procedures.

⛳️ Requirements

• Must be a resident of Oklahoma.

• Hold an active Registered Nurse (RN) license or a Licensed Master Level Social Worker (LCSW, LPC, LMFT).

• Have a minimum of 1 year of clinical experience in Behavioral Health.

• Possess 2 or more years of experience in in-home case or care management.

• Experience in coordinating services, care needs, or benefits for Medicaid and/or Medicare enrollees.

• Familiarity with community health and social service agencies, along with additional community resources.

• Proficient in Microsoft Office applications, including Word, Excel, and PowerPoint.

• Willingness to travel periodically to the Humana Oklahoma City office for meetings and training.

• Case Management Certification (CCM) is preferred.

• Must have a valid driver's license with reliable transportation and the ability to travel within the state.

• Proof of personal vehicle liability insurance with minimum limits of 100,000/300,000/100,000.

• Capability to meet Humana’s home internet standards: a minimum of 25 Mbps download and 10 Mbps upload speed.

• Ability to work from a designated area without distractions to maintain the confidentiality of PHI/HIPAA information.

• Completion of a tuberculosis (TB) screening is required if selected.
