
Care Manager, Behavioral Health

This is a fully remote position, open to applicants in New Jersey.
• Design, evaluate, and implement complex care management initiatives primarily focused on mental and behavioral health requirements.
• Assess member needs through phone consultations or in-home evaluations and recommend or facilitate care/service plans.
• Execute outreach through telephonic, digital, home visits, and other site visits.
• Create continuous care plans for high-acuity members and identify relevant providers, specialists, and community resources.
• Coordinate efforts among members, families/caregivers, community resources, and care provider teams.
• Oversee care plans, monitor member status, treatment side effects, complications, and clinical symptoms.
• Facilitate care coordination and work collaboratively with providers and specialists to ensure timely access to healthcare services.
• Collect, document, and maintain member information and care management activities to comply with regulatory standards.
• Educate members and families about procedures, provider instructions, treatment options, referrals, and healthcare benefits.
• Provide leadership insights on opportunities to enhance care quality and cost-effectiveness.
• Adhere to all policies and standards while performing additional duties as assigned.
• Master’s degree in Behavioral Health or Social Work or a degree from an accredited nursing school.
• 2–4 years of relevant experience.
• Licensed Master’s Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN in accordance with state contract requirements.
• Experience in behavioral health is mandatory.
• For NJ Health Plan: Master’s degree in Behavioral Health or Social Work along with Licensed Associate Counselor (LAC) or Licensed Social Worker (LSW) is required.
• An active New Jersey license is necessary.
• Candidates must reside in Essex or Union County or in a neighboring bordering county.
• Availability is required from Monday to Friday, 9:00 AM to 5:00 PM.
