Care Manager

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Deliver intricate care coordination and monitoring for members with primary Intellectual or Developmental Disabilities, as well as potential secondary needs in mental health, substance abuse, or complex medical/physical health.

• Employ person-centered planning techniques and strategies to collect information and cater to the unique needs of each member.

• Offer education and support regarding the rights and responsibilities of members.

• Conduct discovery activities, information gathering, and assessments as part of the planning process.

• Ensure that assessments and plans are revised as necessary, particularly when members' life situations change.

• Facilitate integrated care management through collaboration across multiple disciplines.

• Oversee the quality of support and compliance with waiver requirements.

• Travel within Trillium's designated catchment area as needed.

⛳️ Requirements

• Must possess QP status.

• Fully licensed by the North Carolina governing board that regulates Human Services professions; OR fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing with four (4) years of full-time experience with the MH/DD/SA population served; OR a master’s degree in a human service field with one year of full-time pre- or post-graduate degree MH/DD/SA experience; OR a bachelor’s degree in a human service field with two years of full-time pre- or post-bachelor's degree MH/DD/SA experience; OR a bachelor’s degree in another discipline with four years of full-time pre- or post-bachelor's degree MH/DD/SA experience.

• Substance abuse professional alternatives necessitate the specified supervised experience in alcoholism and drug abuse counseling.

• For members with LTSS needs: minimum of two (2) years of previous experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management; may coincide with QP experience.

• Valid driver's license required.

• Must reside within 75 miles of Trillium's North Carolina catchment area.

• Ability to travel within the catchment area as necessary.

• Must successfully pass a drug test as a condition of employment.
