This is a fully remote position, open to applicants in New Jersey.
• Design, evaluate, and implement intricate care management processes primarily focused on mental and behavioral health requirements.
• Assess member needs through telephonic communication or in-home assessments and propose or facilitate appropriate care plans.
• Perform outreach via phone, digital platforms, home visits, and other locations.
• Create continuous care plans for members with high acuity.
• Identify healthcare providers, specialists, and community resources pertinent to mental health and substance use disorders.
• Coordinate efforts among members, their families, caregivers, community resources, and care provider teams.
• Oversee care plans, monitor treatment side effects, complications, clinical symptoms, and overall outcomes.
• Promote care coordination and ensure timely access to essential services.
• Gather, document, and uphold member information and care management activities to ensure regulatory compliance.
• Educate members and their families on procedures, provider instructions, treatment options, referrals, and healthcare benefits.
• Provide leadership insights regarding care and quality enhancement initiatives.
• Adhere to policies and standards while executing other assigned responsibilities.
• A Master's degree in Behavioral Health or Social Work, or a degree from an accredited nursing school.
• 2–4 years of relevant experience.
• Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN in accordance with state contract specifications, with required behavioral health experience.
• For NJ Health Plan: A Master's degree in Behavioral Health or Social Work along with a Licensed Associate Counselor (LAC) or Licensed Social Worker (LSW) is mandatory.
• An active New Jersey license is required.
• Must reside in Camden or Gloucester County, or in a neighboring bordering county.
• Availability from Monday to Friday, 9:00 AM to 5:00 PM.
