Care Management Coordinator

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Perform outreach to patients and their families to enhance access to primary care, preventive services, and community resources.

• Assist patients and their families in navigating the healthcare system, scheduling appointments, making referrals, and accessing support services.

• Establish trusting relationships to encourage engagement in care and adherence to treatment plans.

• Identify obstacles to care such as transportation issues, insurance challenges, food insecurity, and housing problems, connecting patients and families with the appropriate resources.

• Deliver culturally relevant health education focused on chronic disease management, preventive health, medication adherence, and overall wellness.

• Support care management personnel and clinical teams through follow-up calls and coordination of care.

• Document patient interactions, services rendered, and outcomes in Electronic Health Records (EHR) or other designated systems.

• Participate in multidisciplinary team meetings and case discussions.

• Uphold confidentiality and maintain professional boundaries while promoting patient empowerment and self-advocacy.

• Engage in continuing education, training, and initiatives for quality improvement.

• Process and monitor incoming and outgoing referrals.

• Adhere to scheduled work hours and maintain reliable attendance.

• Carry out additional duties as assigned.

⛳️ Requirements

• A high school diploma or equivalent is mandatory.

• An associate’s degree in health, human services, or a related field is preferred.

• At least two years of experience in a healthcare or social service environment, ideally in care coordination, case management, or patient navigation.

• Familiarity with healthcare systems, community resources, and social determinants of health.

• Competence in using computers and electronic health record (EHR) systems.

• Excellent communication, organizational, and problem-solving abilities.

• A valid Wisconsin Driver’s License is required, along with an acceptable motor vehicle record (MVR), according to FHC guidelines.

• Bilingual proficiency in English and Spanish is preferred.
