This is a fully remote position, open to applicants in Washington.
• Assist CHPW’s Medicaid, Cascade Select, and Dual Special Needs Plan members in navigating healthcare systems and benefits.
• Perform welcome calls for new members and conduct Health Risk Assessments.
• Engage in routine outreach for annual Health Risk Assessments, health screenings, and participation in Care Management programs.
• Act as the main contact for the Care Team and manage incoming member calls.
• Execute telephonic outreach for welcome calls, satisfaction surveys, and Health Risk Assessments.
• Inform members about benefits, screenings, program requirements, and how to engage with the Care Team.
• Organize and arrange in-home assessments as well as telehealth or in-person provider visits.
• Reach out quarterly to members who were previously unreachable.
• Assist members in closing care gaps and scheduling necessary appointments.
• Triage and resolve inquiries from members, families, and caregivers, or refer them to the appropriate personnel.
• Receive warm handoffs from other internal teams for HRA or Care Team support.
• Distribute educational materials via phone, mail, or text.
• Participate in Care Team meetings and interdisciplinary team conferences.
• Connect members with relevant internal programs and CHPW teams.
• Access and manage medical records, upload necessary documents, and maintain EHR episodes.
• Close EHR cases for members who are no longer enrolled and inform the assigned case managers.
• Activate the Crisis Caller in Distress protocol.
• Prioritize and accomplish daily tasks both independently and collaboratively.
• Carry out additional duties as assigned.
• A minimum of three (3) years of experience in administrative, customer service, and/or patient services coordination within a healthcare environment is required.
• At least one (1) year of experience as a Medical Assistant or in a related field is preferred.
• Certification through an accredited medical assistant program, Allied Health education program, or other relevant certification is preferred.
• Familiarity with medical terminology, electronic medical records, and healthcare processes is preferred.
• Proficiency in Spanish is preferred.
• Capability to meet deadlines, manage time efficiently, and organize and prioritize tasks.
• Strong verbal and written communication skills.
• Competence in using Microsoft Office applications such as Outlook, Word, and Excel.
• Must successfully pass a criminal background check.
• Must not be sanctioned or excluded from participating in federal or state healthcare programs.
• Must comply with vaccination requirements, subject to medical or religious exemptions.
• Ability to perform essential functions with reasonable accommodations as needed.
