Bilingual LPN, Care Navigator

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct phone consultations with patients who are part of care management and/or remote patient monitoring programs.

• Offer support and education regarding patients’ chronic health conditions.

• Inform patients about health conditions as well as lifestyle and behavior strategies.

• Help patients establish and achieve goals that align with provider-approved care plans.

• Keep accurate and current patient records and document interactions and care plans according to protocols.

• Address patient issues and barriers to care, finding practical solutions to enhance adherence and outcomes.

• Communicate patient requirements to providers through established workflows.

• Engage in training sessions, team meetings, and quality improvement initiatives.

• Respond to remotely transmitted patient data such as blood pressure, blood glucose, weight, and pulse oximetry, following approved partner workflows.

• Build relationships with assigned patients to foster chronic illness management and enhance health.

⛳️ Requirements

• Must possess a current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN).

• Additional state licenses may be necessary.

• A minimum of 3 years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring.

• Proficient in verbal and written communication in Spanish.

• Strong communication and interpersonal abilities.

• Excellent organizational and time management skills.

• Proficient in utilizing electronic health records (EHR) and care management software.

• Capability to work autonomously as well as collaboratively within a team.

• Empathetic and patient-centered approach to care.

• Must have a reliable internet connection.

• HIPAA-compliant work environment is required.

• Familiarity with virtual communication tools like Zoom and Slack.
