Bilingual Health Coach, Spanish Speaking

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform outbound telephonic outreach to patients and offer individualized coaching and support for those managing chronic conditions.

• Assist patients in implementing tailored care plans and achieving their goals through education and coaching on behavior modifications, nutrition, physical activity, and self-management.

• Carry out thorough assessments of patients’ health status, lifestyle choices, nutritional habits, and their readiness for change.

• Provide education on disease management, medication adherence, recognition of symptoms, and preventive strategies under suitable clinical supervision.

• Track patient progress, adherence to treatment plans, and health outcomes through consistent check-ins and remote monitoring tools.

• Record patient interactions, outcomes, and follow-up strategies in the Cadence platform.

• Identify obstacles to care and connect patients with resources or internal support pathways.

• Work collaboratively with clinicians and patient support teams utilizing Cadence Platform, Google Workspace, and Slack.

• Offer insights to enhance workflows.

⛳️ Requirements

• A valid NBHWC health coaching certification is mandatory.

• A minimum of 1 year of experience as a health coach is essential.

• Experience in providing patient education and support for individuals managing chronic conditions is necessary.

• Readiness for remote work, including reliable high-speed WiFi, is required.

• Proficiency in Spanish is essential.

• Familiarity with AI-assisted clinical tools such as Gemini, Open Evidence, or similar platforms is required.

• Capability to engage credibly with providers and patients regarding the role of AI in care delivery and the importance of human judgment in critical situations.
