Bilingual Clinical Care RPM LVN

This is a fully remote position, open to applicants in United States.

📋 Description

• Make 70–90 outbound phone calls daily to follow up with patients and address their health issues

• Manage incoming phone calls and direct them based on clinical priority

• Direct non-clinical incoming calls to the relevant company departments

• Oversee and react to Remote Patient Monitoring (RPM) alerts

• Elevate clinical concerns when necessary

• Work in conjunction with providers to ensure timely and effective patient care

• Conduct monthly wellness evaluations and thorough chart reviews

• Precisely record patient interactions in the clinical platform in real time

• Achieve or surpass individual and team performance benchmarks for care quality, patient engagement, response times, and adherence to protocols

• Ensure compliance with company policies and relevant regulations

• Utilize AI tools to enhance clinical documentation and care coordination

• Undertake additional responsibilities as assigned

⛳️ Requirements

• Valid LVN/LPN license is mandatory

• Competent in using computers, EMRs, and telehealth applications

• Excellent communication and organizational abilities

• Proficient in both English and Spanish

• Minimum internet speed of 50 Mbps for downloads and 10 Mbps for uploads

• A hardwired internet connection is essential

• Submission of a speed test is required during the offer process

• A private workspace that complies with HIPAA regulations is necessary

• Availability to work rotating holidays throughout the year is required

• Must cover at least two weekends each month

• Completion of four weeks of training, Monday through Friday, from 9:00 AM to 6:00 PM Eastern Time, with mandatory attendance

• A genuine interest in AI and comfort with using tools such as Claude, ChatGPT, and workflow automation platforms

• Preferred: At least 1 year of nursing experience; experience in RPM, telehealth, or chronic care is advantageous

• Evening Differential: +$1.50/hour for hours worked after 7:00 PM

• Late-Night Differential: +$2.00/hour for hours worked after 10:00 PM

• Weekend Differential: +$1.50/hour for all hours worked on Saturday and Sunday

• Sign-On Bonus: $1,500 paid after 120 days of continuous employment, subject to active employment and satisfactory performance at the time of payout

• Monthly Bonus Potential: Up to $1,500 per month based on patient engagement and performance metrics
