Bilingual Case Management Analyst

This is a fully remote position, open to applicants in Florida.

📋 Description

• Coordinate care to enhance optimal health outcomes for members.

• Educate members and facilitate their access to suitable healthcare services.

• Utilize care management tools and data analysis to evaluate member needs and identify potential risks.

• Create and execute effective care plans.

• Work collaboratively with internal teams, healthcare providers, and external resources to overcome barriers.

• Enhance the quality of care and ensure services are consistent with member benefits.

• Encourage member engagement, informed decision-making, and healthy behavior changes through effective communication, coaching, and motivational strategies.

• Track the progress of care plans and document activities accordingly.

• Ensure adherence to regulatory standards, company policies, and quality management protocols.

• Travel to various locations within Florida up to 75% of the time.

⛳️ Requirements

• Bilingual in English and Spanish is mandatory.

• Must successfully complete both verbal and written language assessments in both languages.

• Must have reliable transportation and be willing to travel up to 75% of the time.

• Must reside in North Miami, Miramar, Aventura, Pembroke Pines, Hialeah, or Flagler, Florida.

• A minimum of 1 year of experience in a behavioral health or long-term care environment.

• Proficient in Microsoft Office Suite and comfortable with computer usage.

• Bachelor’s Degree or a non-licensed Master’s level clinician is required.

• A degree in behavioral health or human services, such as psychology, social work, or marriage and family counseling is necessary.

• Experience in discharge planning, case management, or managed care is preferred.

• Ability to multitask, prioritize, and adapt in a fast-paced setting is preferred.

• Advanced verbal and written communication skills in both English and Spanish are preferred.
