This is a fully remote position, open to applicants in Florida.
• Coordinate care to promote optimal health outcomes for members.
• Provide education and facilitate access to suitable healthcare services.
• Utilize care management tools and data analysis to evaluate member needs and recognize risks.
• Create and execute effective care plans.
• Work collaboratively with internal teams, providers, and external resources to overcome barriers.
• Enhance the quality of care and ensure services are in line with member benefits.
• Encourage member engagement, informed decision-making, and healthy behavior changes through communication, coaching, and motivational techniques.
• Track the progress of care plans.
• Document all activities.
• Ensure adherence to regulatory standards, company policies, and quality management guidelines.
• Bilingual (English & Spanish) is mandatory.
• Must successfully complete both verbal and written language assessments.
• Reliable transportation is required.
• Willingness and ability to travel up to 75% of the time is necessary.
• Must reside in Miami-Dade County, FL.
• A minimum of 1 year of experience in a behavioral health or long-term care environment.
• Proficiency in Microsoft Office Suite and general computer skills is essential.
• A Bachelor’s Degree or a non-licensed Master’s level clinician is required.
• A degree in behavioral health or human services, such as psychology, social work, or marriage and family counseling is necessary.
