Bilingual Care Coordinator – Full-Time

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct monthly phone calls for CCM with the designated practice.

• Place 30–50 outbound calls each day.

• Fulfill necessary patient interactions to achieve CCM service objectives.

• Provide education to patients regarding chronic conditions and their care plans.

• Facilitate coordination of care with provider offices, pharmacies, and community resources.

• Recognize gaps in care as well as social determinants of health.

• Develop and update tailored care plans within the EMR system.

• Ensure accurate documentation of all patient interactions.

• Uphold HIPAA compliance and maintain patient confidentiality.

⛳️ Requirements

• A minimum of 1-3 years of experience as a CMA/RMA or LPN.

• Active certification or multistate license for LPNs.

• Excellent verbal and written communication skills in English.

• Comfortable with being on the phone for the majority of the day.

• Experience with EMR/EHR systems.

• Highly organized, dependable, and capable of working independently in a remote environment.

• A private, quiet home office free from interruptions.

• Reliable high-speed internet connection (100 Mbps or higher).

• A second monitor is required.

• Preferred: Experience with geriatric patients.

• Preferred: Background in care coordination or case management.

• Preferred: Strong verbal and written communication skills in Spanish.
