Bilingual Care Coordinator – Full-Time

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct monthly phone consultations with the designated practice.

• Make between 30 to 50 outbound calls each day.

• Fulfill necessary patient interactions to achieve CCM service objectives.

• Provide education to patients regarding chronic illnesses and their care plans.

• Facilitate coordination of care with provider offices, pharmacies, and community resources.

• Identify care gaps and social determinants affecting health.

• Develop and revise personalized care plans within the EMR system.

• Precisely document all interactions with patients.

• Ensure compliance with HIPAA regulations and maintain patient confidentiality.

• Dedicate 7 to 8 hours each day to phone interactions.

• Adhere to a structured full-time schedule of 40 hours per week, from Monday to Saturday, 9:30 AM to 6:00 PM in the relevant time zone.

• Achieve monitored productivity and quality standards.

⛳️ Requirements

• A minimum of 1 to 3 years of experience as a CMA/RMA or LPN.

• Current certification or multistate license for LPNs.

• Excellent verbal and written communication skills in English.

• Comfortable with spending most of the day on the phone.

• Experience with EMR/EHR systems is required.

• Must be organized, dependable, and capable of working independently in a remote environment.

• Preferred: experience with geriatric patients.

• Preferred: background in care coordination or case management.

• Preferred: proficiency in verbal and written communication in Spanish.

• A private, quiet home office environment free from interruptions.

• Access to reliable high-speed internet (100 Mbps or higher).

• A second monitor is mandatory for this role.
