Bilingual Cardiac Rehab Intake Specialist – Contractor

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform initial evaluations for patients joining the virtual cardiac recovery program.

• Inform patients about the advantages of the program and the significance of modifying risk factors.

• Guide individuals through electronic chat messaging and video conferencing.

• Assist individuals in resolving issues and achieving their current and evolving objectives.

• Modify in-clinic best practices to fit a remote care delivery model.

• Develop personalized treatment plans, encompassing assessments of nutrition, psychosocial factors, fitness, and co-morbidities.

• Convert existing evidence-based best practices for chronic condition management into a relatable and empowering format.

• Inspire and support patients during their initial consultations.

• Review patients' medical histories.

• Establish goals and conduct motivational interviewing for specific populations.

• Collaborate with exercise physiologists, patient experience teams, product teams, and marketing departments.

• Monitor individual progress and provide feedback and support to patients.

⛳️ Requirements

• At least three years of experience assisting individuals in managing their health, preferably in a clinical or remote health coaching environment.

• A degree in a health-related discipline.

• Background in counseling or education regarding disease management within a medical context, along with familiarity with recent nutrition and exercise trends.

• Proficiency in English, both written and spoken.

• Bilingual in English and Spanish is required.

• Some experience in a clinical environment.

• Basic knowledge of cardiac rehabilitation.

• Experience working with elderly patients.

• Fundamental understanding of cardiac procedures and medications.

• Basic knowledge of clinical documentation practices.

• Experience in coaching for smoking cessation, diabetes management, and special populations is a plus.

• Familiarity with the management of cardiac conditions, CHF, other chronic illnesses, smoking cessation, women’s health, substance abuse, mental health, or contemporary nutrition and exercise trends.
