This is a fully remote position, open to applicants in New York.
• Provide case management for behavioral health and substance abuse disorders via telephone and/or home visits within the limits of licensure.
• Address complex cases and specific requests related to accounts.
• Utilize knowledge of screening criteria and clinical judgment to evaluate member needs.
• Perform assessments and create personalized care plans that target identified objectives and goals.
• Monitor and assess the effectiveness of care plans, making modifications as necessary.
• Facilitate member access to suitable, high-quality, and cost-effective care.
• Coordinate with both internal and external resources to fulfill members' identified needs.
• Work collaboratively with healthcare providers.
• Act as a resource for other Behavioral Health Case Managers.
• Engage in cross-functional team projects and initiatives.
• Serve as a subject matter expert in specific clinical areas such as Eating Disorders, Maternity, Alcohol/Drug issues, and Autism Spectrum Disorders.
• A Master's degree (MA/MS) in social work, counseling, or a related behavioral health discipline, or a nursing degree, along with a minimum of 3 years of clinical experience in social work or counseling, with extensive experience in intricate psychiatric and substance abuse disorder treatment; or a comparable combination of education and experience.
• A current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist, as permitted by applicable state laws, to practice as a health professional within the scope of licensure in the relevant states or territories of the United States.
• Must be licensed in New York State.
• Prior experience in case management and telephonic and/or face-to-face coaching with members experiencing complex psychiatric, substance abuse, and/or medical disorders.
• Experience in managed care is required.
• For Government business only: LAPC or LAMFT may be accepted as authorized by applicable state laws, along with any other relevant state or federal requirements.
• Experience in health coaching and motivational interviewing techniques is preferred.
• Managed Care/Insurance experience is preferred.
• Familiarity with community resources in the New York City/Westchester area is preferred.
• Ability to work virtually full-time within a reasonable commuting distance from the posting location(s), unless an accommodation is provided.
• Availability to attend mandatory in-person training sessions.
• May be required to visit members in person based on business needs.
