Behavioral Health Case Management Coordinator

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Conduct behavioral health case management in line with established department standards.

• Collaborate with physicians, providers, members, and other available resources.

• Evaluate, plan, facilitate, coordinate, monitor, and advocate for health options and services.

• Enhance quality and cost-effective results through effective communication and resource utilization.

• Act as a liaison among providers, physicians, and members.

• Represent the efforts of the dedicated custom team during client audits.

• Occasionally deliver presentations onsite at the Downers Grove office.

⛳️ Requirements

• Registered Nurse (RN) OR a Master's-level Behavioral Health Professional or Doctorate in a Behavioral Health field (LCSW, LPC, LCPC, LMFT, RPsy) with a license for independent practice.

• Possess a current, valid, and unrestricted clinical license in the state of operation (IL).

• Minimum of 2 years of clinical experience post-licensure at the independent practice level delivering direct clinical care to consumers.

• Experience in utilization management, discharge planning, or managed care.

• Strong customer service orientation.

• Proficient in PC and database applications.

• Excellent verbal and written communication skills.

• If a Registered Nurse, at least 2 years of psychiatric clinical experience post-licensure providing direct clinical care to consumers.

• Case Management Certification (CCM) is preferred.

• Preferred: 3 years of clinical practice experience.

• Familiarity with ancillary services such as wellness or community-based programs.

• Preferred: Provider-side insurance experience.

• Preferred: Knowledge of Utilization Management or Case Management activities and standardized criteria sets.

• Preferred: Bilingual in English/Spanish or English/Polish.

• Experience interfacing with clients, strong communication and presentation abilities, along with problem-solving skills.

• Proficient in Microsoft Teams and virtual training/productivity tools.

• Ability to collaborate effectively with an interdisciplinary team and adapt to changing circumstances.

• Must be available to work during business hours from 8:00 AM–5:00 PM CST, Monday through Friday.
