Behavioral Health Care Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Execute essential Behavioral Health Care Manager duties within the Collaborative Care Model program, adhering to the guidelines set forth by the University of Washington AIMS Center.

• Assist in the mental and physical health management of designated patients in collaboration with healthcare providers and mental health specialists.

• Evaluate and assess patients for mental health conditions and promote engagement and follow-up care.

• Inform patients and their caregivers about mental health conditions and the treatment options available.

• Track clinical symptoms and treatment results, document advancements, and modify care plans under supervision.

• Provide evidence-based behavioral interventions, such as motivational interviewing, behavioral activation, and problem-solving therapies.

• Engage in case reviews with psychiatric consultants and licensed neuropsychologists.

• Coordinate referrals to social services, vocational rehabilitation, and specialized care.

• Keep precise records in Synapticure’s EHR system and maintain communication with team members involved in patient care.

• Aid in the development of relapse prevention strategies.

• Cultivate relationships with patients, caregivers, and partners to promote trust and ensure continuity of care.

⛳️ Requirements

• Possession of an active independent clinical license (LCSW, LMFT, LPC, or equivalent) allowing for independent practice.

• 0-2 years of experience in clinical case or care management.

• Excellent organizational, communication, and interpersonal abilities.

• A strong commitment to working with vulnerable populations and delivering patient-centered care.

• Preferred background or clinical interest in ALS, Parkinson’s Disease, Dementia care, or other neurodegenerative or chronic disease management.

• Familiarity with insurance protocols and medical billing/coding is advantageous.

• Experience in reviewing medical records and exposure to clinical trials is preferred.

• Demonstrated ability to adapt and excel in a startup or rapidly expanding organization is preferred.

• Knowledge of current trends in neurodegenerative diseases, terminal illnesses, targeted therapies, and clinical trials is desirable.

• Must be eligible to work in the United States; Synapticure cannot provide work visa sponsorship.
