This is a fully remote position, open to applicants in Kentucky, +4 more states.
• Design, evaluate, and implement intricate care management initiatives for members primarily facing mental and behavioral health challenges.
• Assess member requirements through phone consultations or in-home visits and propose or facilitate care/service plans.
• Execute outreach via telephone, digital platforms, home visits, and other locations.
• Create continuous care plans for high-acuity members.
• Identify healthcare providers, specialists, and community resources for mental health and substance use disorder support.
• Coordinate efforts among members, families/caregivers, community resources, and care provider teams.
• Monitor care plans, member conditions, treatment side effects, complications, and clinical symptoms.
• Facilitate care coordination and collaborate with providers and specialists to guarantee timely access to care.
• Gather, document, and maintain member information and care management activities to ensure regulatory compliance.
• Educate members and their families regarding procedures, provider directives, treatment alternatives, referrals, healthcare benefits, and social determinant requirements.
• Offer leadership insights on opportunities to enhance cost-effective care and quality service delivery.
• Adhere to all policies and standards while performing other assigned duties.
• A Master's degree in Behavioral Health or Social Work, or a degree from an accredited nursing program.
• 2–4 years of relevant experience.
• Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN in accordance with state contract requirements, with mandatory behavioral health experience.
• Preference given to candidates with an active independent Kentucky licensure, particularly LCSW or LPCC.
• Experience in case management and mental health services, including serious mental illness (SMI) or substance use disorders.
• Residency in Kentucky or neighboring states such as Ohio, Indiana, West Virginia, or Tennessee is preferred.
• Ability to work Monday through Friday, from 8:00 AM to 5:00 PM Eastern Time.
