Weekend Virtual Care Nurse

This is a fully remote position, open to applicants in Florida.

📋 Description

• Conduct virtual consultations and clinical evaluations for patients

• Carry out triage procedures and identify preliminary warning signs or risk patterns

• Assist in managing chronic illnesses and educating patients

• Oversee telehealth data and raise concerns when necessary

• Aid in the prevention of unnecessary emergency room visits and readmissions

• Collaborate with interdisciplinary healthcare teams

• Record patient interactions accurately and in a timely manner

• Engage in care planning and initiatives for performance enhancement

• Contribute to the development of best practices for virtual care

• Provide proactive remote assistance and clinical intervention to decrease hospitalizations and enhance patient outcomes

⛳️ Requirements

• Valid, unrestricted Florida LPN license

• At least 2 years of experience in home health

• Background in triage and prevention of hospitalizations

• Strong clinical judgment and effective communication skills

• Proficient in using telehealth technologies and EMR systems

• Must have a reliable internet connection and a private workspace

• Candidates MUST reside in Florida

• Employment is contingent upon passing an AHCA Level 2 background check

• Preferred: Experience with high-risk or medically complex patients

• Preferred: Familiarity with chronic disease management

• Preferred: Proficiency in Microsoft Office
