Virtual Seasonal Advanced Practice Provider – NP/PA

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct primarily telehealth video consultations, with occasional and limited in-home visits to members within the local area.

• Execute thorough assessments and deliver evidence-based care aimed at prevention, chronic condition management, medication optimization, and health maintenance.

• Identify and resolve care gaps.

• Collaborate with both external and internal care team members.

• Contribute to personalized, holistic care plans.

• Leverage integrated technology and AI-driven workflows to minimize administrative tasks and enhance meaningful interaction time with members.

• Communicate with empathy and clarity, building trust and confidence with members and their caregivers.

• Work alongside primary care providers, specialists, pharmacists, nurses, care coordinators, caregivers, and families to ensure coordinated, member-focused care.

⛳️ Requirements

• Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license.

• A minimum of 3 years of outpatient clinical experience, preferably in primary care, family medicine, internal medicine, or geriatrics.

• Active and clear NP/PA license.

• Willingness to obtain additional licenses if required within the first 90 days.

• Current BLS certification at the time of hire.

• Comfortable delivering care via video telehealth and utilizing electronic documentation systems.

• Ability to commit to a full-time (40 hours/week) schedule for the entire seasonal assignment (November through March 31st).

• Familiarity with managed care models, including STARS/HEDIS and the identification of care gaps.

• Experience in conducting comprehensive or preventive care visits for Medicare populations.

• Experience in supporting older adults and/or individuals with complex medical or social needs.
