Virtual Primary Care Nurse Practitioner

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver evidence-based virtual primary care for individuals aged 18 and above.

• Conduct thorough, accurate, concise, relevant, and age-appropriate history and physical evaluations.

• Assess and address patients’ medical requirements with safe, high-quality care.

• Provide guidance and education to patients and their families.

• Record patient care in the electronic health record in accordance with existing policies and procedures.

• Collaborate with a dedicated care team along with internal and external colleagues to coordinate patient care.

• Offer compassionate, suitable, and effective patient-centered care.

• Advocate for health promotion, disease prevention, and treatment.

• Refer patients to the appropriate specialists and care components.

• Implement quality measurement awareness in managing chronic diseases.

• Document patients wishing to join the care team panel using approved EMR workflows.

• Engage in or lead practice committees such as Epic Optimization and Care Pathways.

⛳️ Requirements

• Active Advanced Practice Registered Nurse (APRN) or Nurse Practitioner (NP) license in good standing.

• Current National Board Certification in Family Practice.

• Over 5 years of recent experience as a Nurse Practitioner in primary care or family medicine.

• Strong verbal, written, and electronic communication skills.

• Exceptional organizational abilities and capacity to manage multiple tasks.

• Initiative, problem-solving capabilities, adaptability, and flexibility.

• Capacity to work remotely within a care team environment.

• Proficiency in information management and technology.

• Obtain and maintain multi-state licensure in good standing.

• Willingness to take after-hours and/or overnight calls to support patient care.

• Completion of a master’s degree level Family Nurse Practitioner program.

• Current National Board Certification and state license to practice as an Advanced Practice Nurse.

• Access to broadband connectivity, a quiet environment with a neutral background, and ability to maintain patient privacy according to CVSH guidelines.

• Capability to interact remotely with patients, including sitting, writing, operating a computer and phone, speaking clearly, and hearing patient responses.

• Ability to view and read a computer screen and other electronic devices.
