Case Management Coordinator – Inpatient

This is a fully remote position, open to applicants in United States.

📋 Description

• Initiate the discharge planning process upon receiving patient admission notifications.

• Engage with patients in inpatient or post-acute settings and evaluate their readiness for discharge.

• Collaborate with Hospitalists, discharge planners, and payer care teams to formulate discharge plans.

• Coordinate timely follow-up appointments with primary care providers and specialists.

• Educate patients and their families about care instructions, available resources, and home-based options.

• Identify and resolve barriers to safe discharge, including transportation and social needs.

• Document interactions, plans, and care coordination activities within the electronic medical record (EMR).

• Monitor and communicate patient outcomes and the effectiveness of care to interdisciplinary teams.

• Facilitate communication among providers, facilities, and community-based organizations.

• Contribute to initiatives aimed at improving processes related to care transitions.

⛳️ Requirements

• High school diploma or equivalent.

• 3–5 years of experience in clinical, case management, or community health environments.

• Experience in inpatient discharge planning or care coordination.

• Medical Assistant License (mandatory).

• Excellent communication, counseling, and documentation abilities.

• Proficiency in EMR systems, Microsoft Office, and clinical tools.

• Familiarity with medical terminology and post-acute care services.

• Understanding of healthcare navigation and social determinants of health.

• Capacity to work effectively both collaboratively in teams and independently.

• Advanced proficiency in English.

• Willingness to travel to designated centers and company locations as required.

• Capability to sit for extended periods, utilize a computer, occasionally stand and walk, and lift or carry up to 20 pounds.
