Virtual Physician – Transitions of Care, Hospitalist / Post-Acute

This is a fully remote position, open to applicants in United States.

📋 Description

• Provide exceptional direct virtual patient care to patients recently discharged who are at a high risk for readmission.

• Act as the ultimate escalation point for clinical inquiries and complex patient management decisions.

• Monitor quality and clinical outcome metrics that contribute to improved readmission rates.

• Offer supervision, instruction, and mentorship to Nurse Practitioners through collaborative practice agreements across various states.

• Conduct weekly interdisciplinary rounds and readmission meetings.

• Assess high-risk patients and intricate cases while performing root cause analyses.

• Deliver clinical oversight and guidance across multiple disciplines.

• Collaborate with operational leaders and senior management for nursing, medical assistants, and clinical coordinators.

• Promote a patient-centered and transparent clinical culture within a distributed team.

• Recognize readmission trends and formulate strategies to avert hospital readmissions.

• Lead quality improvement initiatives and process enhancements.

• Utilize and provide insights on Devoted's Orinoco electronic health information system.

• Contribute to the creation, education, and implementation of AI-enhanced clinical tools.

• Champion healthcare equity and accessibility.

• Manage oversight through collaborative practice agreements across various states.

⛳️ Requirements

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.

• Board certification in Internal Medicine, Family Medicine, or a related specialty is necessary.

• A minimum of 3 years of post-residency clinical experience is required.

• Strong experience in inpatient care and a comprehensive understanding of the transitions of care period.

• Proficient in managing chronic medical conditions such as heart failure, COPD, diabetes, hypertension, pneumonia, cellulitis, and urinary tract infections.

• Experience in leading interdisciplinary rounds and conducting thorough root cause analyses.

• Direct experience in supervising Nurse Practitioners or other healthcare professionals is essential.

• Demonstrated ability to manage change, translate vision and strategy to front-line teams, and foster innovation in a dynamic healthcare setting.

• Strong analytical and problem-solving skills with a background in using data to drive clinical and operational enhancements.

• Collaborative approach with the ability to work effectively with interdisciplinary teams and business partners.

• Active medical license in good standing is required.

• Willingness to obtain licensure in additional states as the program grows.

• Background in hospital medicine, post-acute care, primary care, or geriatrics is preferred.

• Previous experience in a medical director or similar physician leadership position within value-based care or Medicare Advantage organizations is advantageous.

• Experience providing care in a remote/virtual setting or utilizing telehealth platforms is beneficial.

• Familiarity with quality improvement methodologies and leading clinical initiatives is a plus.

• Experience within rapidly expanding healthcare organizations is desirable.

• Experience serving diverse patient populations is a plus.
