Virtual Neurology Care Nurse Practitioner

This is a fully remote position, open to applicants in Virginia.

📋 Description

• Deliver direct clinical services to patients experiencing neurological disorders, memory issues, and/or dementia through telehealth.

• Perform assessments of patients.

• Create personalized care plans.

• Oversee ongoing treatment in partnership with supervising physicians.

• Collaborate with neurologists and the wider care team to maintain continuity of care.

• Inform patients and their families about diagnoses, treatment strategies, and disease progression.

• Ensure precise and timely clinical documentation in the EMR in accordance with regulatory standards.

⛳️ Requirements

• Residency in Virginia is required.

• Must possess an active, unrestricted Nurse Practitioner license in Virginia.

• Geriatrics, adult-gerontology, or family practice certification is preferred.

• A minimum of 2 years of clinical experience in neurology, memory care, and/or dementia care environments.

• Strong clinical judgment and documentation skills are essential.

• Ability to manage a complete patient panel independently, while collaborating with physicians.

• Exceptional communication skills, particularly with patients experiencing cognitive decline and their caregivers.
