Virtual Neurology Care Nurse Practitioner

This is a fully remote position, open to applicants in Connecticut.

📋 Description

• Deliver direct clinical care to patients experiencing neurological disorders, memory issues, and/or dementia through telehealth services.

• Perform patient evaluations, create care plans, and oversee ongoing treatment in collaboration with supervising physicians.

• Collaborate with neurologists and the wider care team to ensure seamless continuity of care.

• Inform patients and their families about diagnoses, treatment strategies, and the progression of diseases.

• Uphold precise and timely clinical documentation in the EMR in accordance with regulatory standards.

⛳️ Requirements

• Must reside in Connecticut.

• Possess an active, unrestricted Nurse Practitioner license in Connecticut.

• Geriatrics, adult-gerontology, or family practice certification is preferred.

• A minimum of 2 years of clinical experience in neurology, memory care, and/or dementia care environments.

• Strong clinical judgment along with proficient clinical documentation abilities.

• Comfortable managing a full patient panel independently while collaborating with physicians.

• Exceptional communication skills when interacting with patients experiencing cognitive decline and their caregivers.
