This is a fully remote position, open to applicants in Philippines.
β’ Submit outbound requests for records to hospitals, imaging centers, and specialist offices.
β’ Monitor pending requests, perform follow-ups, and address any missing documentation.
β’ Upload, index, and categorize incoming medical records into the appropriate Epic patient chart.
β’ Confirm patient demographics, direct records to attending cardiologists, and eliminate duplicate entries.
β’ Organize and file cardiology records, which include consult notes, discharge summaries, EKGs, echocardiograms, stress tests, Holter/event monitor logs, cardiac catheterization reports, and lab results.
β’ Oversee the entire medical records lifecycle: requesting, tracking, following up, receiving, verifying, filing, and completing records.
β’ Identify, categorize, and archive cardiology reports, including ECGs, echocardiograms, cardiac CT/MRI, angiograms, and device checks.
β’ Process incoming and outgoing records requests while verifying signed HIPAA release authorizations.
β’ Document released items and escalate legal subpoenas or unusual requests according to established protocol.
β’ Communicate via phone and email with external hospital HIM departments, imaging facilities, testing labs, and clinics.
β’ Utilize Epic work queues, internal notes, document categorization modules, and routing tools to ensure clean chart records.
β’ A minimum of 2+ years of direct experience in supporting a U.S.-based medical practice or outpatient clinic.
β’ Proven experience in requesting, tracking, retrieving, and filing medical records within an EMR system.
β’ A thorough understanding of HIPAA privacy rules, Release of Information guidelines, and patient consent requirements.
β’ Demonstrated capability to independently track a high volume of open requests through to completion with meticulous follow-up habits.
β’ Excellent written and spoken English skills for professional communications via phone and email with hospital health information management (HIM) departments.
β’ A quiet, private, HIPAA-compliant home office equipped with a reliable computer (PC/Mac), high-speed internet, backup power/internet, and a clear headset.
β’ Recent hands-on experience navigating Epic EHR (chart navigation, document upload/indexing, work queues, routing) is strongly preferred.
β’ Previous experience in Cardiology or high-volume specialty practice workflows is strongly preferred.
β’ Prior experience working within hospital records departments, HIM units, or dedicated ROI teams is strongly preferred.
