Triage Registered Nurse, Hospice

This is a fully remote position, open to applicants in Alabama.

📋 Description

• Conduct initial, thorough, and ongoing evaluations of patients and their families to identify hospice requirements.

• Provide comprehensive physical assessments and medical histories of current and past illnesses via phone.

• Deliver professional nursing care by applying all facets of the nursing process.

• Assess and monitor patient conditions.

• Utilize health assessment data to establish nursing diagnoses.

• Create care plans with objectives grounded in nursing diagnoses and palliative care interventions.

• Involve patients and their families in the care planning process.

• Initiate preventive and rehabilitative nursing actions.

• Administer medications and treatments as outlined in the physician’s care plan.

• Provide guidance to patients and families regarding nursing and associated needs.

• Offer health care instructions in accordance with the assessment and care plan.

• Assist patients with daily living activities and promote self-sufficiency and comfort care.

• Serve as case manager and coordinate care for an assigned patient caseload.

⛳️ Requirements

• Must possess a valid RN license in the state of California.

• A minimum of two years of experience is required, with hospice experience being essential.

• Experience with Home Care Home Base is mandatory.

• Must maintain current CPR certification.

• Exhibit exceptional observation, problem-solving, verbal, and written communication skills.

• Nursing skills as per competency checklist.

• Capability to independently organize and prioritize workload.

• Ability to lift, position, and/or transfer patients.

• Ability to lift supplies and equipment.

• Visual acuity and hearing to effectively perform necessary nursing skills.

• Self-motivated and capable of working with minimal supervision.

• Prolonged or significant walking or standing may be required.

• Significant reaching, stooping, bending, kneeling, and/or crouching may be necessary.
