Virtual Clinician, Nurse Practitioner or Physician Assistant

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Conduct online Video Visits lasting approximately 30 minutes with members enrolled in Medicare Advantage, Medicaid, and specific commercial plans.

• Review patients' medications and medical backgrounds.

• Provide education to members regarding their health and address any inquiries they may have.

• Assist in closing care gaps and connecting members to suitable healthcare services.

• Work from a private location equipped with a reliable internet connection.

• Take part in regular meetings with managers, teams, and clinical staff.

• Achieve operational, clinical, and member experience KPIs and metrics established by Signify Health.

• Deliver only telehealth services.

• Utilize a computer, phone, iPad, and other electronic devices while engaging with members remotely.

⛳️ Requirements

• Must be a full-time, board-certified nurse practitioner or physician assistant.

• Possess active state licenses in the areas of coverage.

• Capable of obtaining and maintaining multi-state licenses as necessary in the coverage area.

• FNP or AGNP certification is preferred for nurse practitioners.

• Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth delivery, or similar formats is preferred.

• Must be trained and credentialed for all health plan clients and lines of business in primary coverage state(s).

• Maintain training, affiliation, and compliance with Video Visit requirements.

• Availability for 40 hours per week is required.

• Consistently achieve 320+ capacity units monthly.

• Consistently complete 220+ visits on a monthly basis.

• Must have broadband connectivity with both video and audio capabilities.

• A quiet environment with a neutral background is necessary for conducting visits.

• Ability to maintain health plan member privacy according to CVS Health guidelines.

• Strong verbal, written, and electronic communication skills are essential.

• Ability to work independently without direct supervision and practice autonomously.

• Proficiency in information management and technology is required.
