Virtual Clinician – Nurse Practitioner or Physician Assistant

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Conduct online Video Visits lasting approximately 30 minutes with members enrolled in Medicare Advantage, Medicaid, and selected commercial plans.

• Review patients' medications and medical histories during these Video Visits.

• Provide education to members regarding their health and respond to their inquiries.

• Assist in closing care gaps and guide members to the appropriate care resources.

• Operate from a private area equipped with a reliable internet connection.

• Offer 40 hours of availability each week.

• Deliver consistently over 320 capacity units monthly.

• Complete and maintain a pace of over 220 visits each month.

• Uphold training, affiliation, and compliance with Video Visit protocols.

• Engage in regular meetings with managers, teams, and clinical staff.

• Achieve operational, clinical, and member experience Key Performance Indicators (KPIs) and metrics set by Signify Health.

• Conduct Video Visits exclusively as telehealth services.

• Utilize computers and phones while interacting remotely with health plan members.

⛳️ Requirements

• Must be a full-time, board-certified nurse practitioner or physician assistant.

• Active state licenses required in relevant coverage areas.

• Nurse practitioners should preferably hold FNP or AGNP certification.

• Board certification is mandatory.

• Capability to obtain and sustain multi-state licenses in coverage areas as necessary.

• Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format is preferred.

• Availability for 40 hours per week is essential.

• Capacity to achieve 320+ units monthly and complete 220+ visits each month.

• Maintain training, affiliation, and compliance with Video Visit standards.

• Must be trained and credentialed for all health plan clients and lines of business in primary coverage state(s).

• Must have broadband connectivity with video and audio capabilities.

• A quiet environment with a neutral background is required for conducting visits.

• Ability to maintain health plan member privacy according to CVS Health guidelines.

• Strong verbal, written, and electronic communication skills are essential.

• Must be able to work independently without direct supervision and practice autonomously.

• Proficiency in information management and technology is required.
