Virtual Clinician – Home Evaluations Nurse Practitioner / Physician Assistant

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Conduct online Video Visits lasting approximately 30 minutes with members of Medicare Advantage, Medicaid, and select commercial plans.

• Review patients' medications and medical histories during these Video Visits.

• Educate members regarding their health and address their inquiries.

• Assist in closing care gaps and connecting members with the necessary care services.

• Provide 40 hours of availability each week.

• Maintain a capacity of 320+ units per month.

• Ensure a completion rate of over 220 visits monthly.

• Adhere to training, affiliation, and compliance standards for Video Visit protocols.

• Engage in regular meetings with managers, team members, and clinical staff.

• Achieve Signify Health's operational, clinical, and member experience KPIs and metrics.

• Focus exclusively on telehealth services through Video Visits.

⛳️ Requirements

• Availability for full-time work.

• Board certification as a nurse practitioner or physician assistant.

• Active state licenses in the relevant coverage areas.

• FNP or AGNP certification is preferred for nurse practitioners.

• Capability to obtain and maintain multi-state licenses in the designated coverage areas as needed.

• Complete training and credentialing for all health plan clients and lines of business in primary coverage state(s).

• Maintain current board certification.

• Availability of 40 hours weekly.

• Consistently achieve 320+ capacity units per month.

• Regularly complete 220+ visits monthly.

• Engage in ongoing training and compliance with Video Visit standards.

• Have broadband connectivity with enabled video and audio capabilities.

• Ensure a quiet environment with a neutral background for conducting visits.

• Ability to maintain health plan member privacy in accordance with CVS Health guidelines.

• Strong verbal, written, and electronic communication skills.

• Ability to work autonomously without direct supervision.

• Proficiency in information management and technology systems.

• Capability to interact remotely with members, including sitting, writing, operating a computer and phone, speaking clearly, and hearing member responses.

• Ability to view and read content on computer screens, iPads, and other electronic devices.
