Virtual Care Nurse Practitioner

This is a fully remote position, open to applicants in Virginia, +1 more state.

📋 Description

• Provide comprehensive, evidence-based care utilizing a remote technology platform.

• Evaluate, diagnose, treat, and manage various health conditions.

• Offer health counseling and care planning for patients aged 18 months and older.

• Deliver education and treatment related to pregnancy prevention, STI prevention, and safer sex practices.

• Assess patients in primary care, acute care, chronic conditions, and transitional care.

• Offer counseling and medication management for contraceptive care.

• Educate patients on health maintenance and address patient care inquiries.

• Maintain thorough documentation of all patient care in an EHR according to MinuteClinic standards.

• Collaborate on patient care with both internal and external colleagues, as well as the broader patient-centered medical home.

• Function effectively within a care team, including Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare professionals.

• Work autonomously, prioritize tasks, solve problems, take initiative, and advocate for patients and the practice.

⛳️ Requirements

• Valid, unrestricted license in one of the following states: CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, WV.

• Willingness and capability to obtain additional licensure as necessary.

• At least two years of relevant medical experience or equivalent qualifications.

• Strong verbal, written, and electronic communication skills.

• Exceptional organizational skills with the ability to manage multiple tasks.

• Initiative, problem-solving skills, adaptability, and flexibility are essential.

• Capacity to work independently and practice autonomously.

• Proficient in managing information and utilizing technology effectively.

• Ability to collaborate with professional colleagues to ensure quality patient care.

• Depending on the market, proficiency in additional languages, such as Spanish, may be required.

• Successful completion of a Master’s Degree level Family Nurse Practitioner program.

• Current National Board Certification is required.

• Active State of Employment license to practice in the Advanced Practice Nurse role.

• Reliable broadband connectivity and a quiet environment with a neutral background for remote consultations.

• Capacity to maintain patient privacy according to CVSH guidelines.

• Ability to engage remotely with clients, operate computers and phones, communicate clearly, and comprehend patient feedback.

• Competence in viewing and interpreting information on computer screens and electronic devices.
